Job SummaryThe Director of Post-Acute Quality, Safety and
Reliability oversees and coordinates the comprehensive Clinical
Quality and quality-related programs for the Post-Acute Division,
working collaboratively with the Post-Acute and Quality leadership
teams across Prisma Health. The Director provides oversight and
ongoing performance monitoring and action planning on key
standardized quality and safety metrics. The Director leads the
efforts to develop and implement standardized plans, processes,
structures, strategies and tools for planning and coordinating the
daily operations of the Post-Acute Quality strategy. The position
implements standard work to help ensure continual compliance with
regulations and standards set for by the Joint Commission and other
accrediting bodies. The director supports a comprehensive program
to improve patient outcomes and minimize patient harm through the
implementation of processes and systems that support a consistent,
standardized culture of quality and safety.
The Director, in collaboration with individual post-acute and
service linety leaders, oversees the standard work and day-to-day
operations of the Post-Acute team, influencing and facilitating the
utilization of evidence based practices and aspects of ongoing
performance improvement for physicians and team members. The
position ensures that communication venues and committee forums are
in place to establish mechanisms for accountability, reporting and
information sharing. The leader is responsible for overseeing and
coordinating the achievement of clinical improvement goals for the
full Post-Acute care continuum of Prisma Health. The position
adopts high reliability principles in a data driven culture to
elevate clinical quality performance to best in class.
The scope of oversight includes, but is not limited to, the work
products / functions of Quality Standards, Patient Safety, Clinical
Performance Improvement, Infection Prevention, Accreditation and
Regulatory Readiness, Risk Management and local Clinical Data
Analytics / Abstraction / Reporting operations.Direct the
day-to-day operations and administrative functions in all areas
related to Post-Acute clinical quality.
Serve as a liaison to Medical staff / departments for coordination
of quality improvement actions involving patient care processes,
physician processes, peer review and ongoing care delivery
Monitor Post-Acute quality-related performance metrics, looking for
opportunities, patterns and trends and appropriately championing
teams and driving action plans.
Apply current knowledge of The Joint Commission and other relevant
regulatory and reporting standards in establishing and monitoring a
state of continual readiness.
Oversee Post-Acute quality-related data collection, aggregation and
data entry based on approved criteria and indicators for quality,
patient safety, risk management and other areas.
Participate in the scheduled review and evaluation of the
Post-Acute quality, safety & performance improvement plan (QAPI)
for each individual service-line.
Establish local partnerships with physicians, nurses, and team
members and external partners across all markets in efforts to
enhance communication around Quality, Safety and Experience-related
Monitor Post-Acute performance in national initiatives such as CMS
pay for performance programs and other external designations.
Remain current within post-acute healthcare trends, patterns and
Facilitate the standardization of processes, systems and tools to
reduce variation and drive improvement, while identifying
Post-Acute specific needs.
Serve as the Post-Acute point of contact for Quality and develop
expertise and proficiency as a subject matter expert in quality and
safety related topics and high reliability.
- Education - Masters degree in a clinical or healthcare related
field required (i.e., health care administration, nursing, public
health, pharmacy, respiratory therapy, etc.)
- Experience - Minimum of 7 years related experience required
(i.e., Quality Standards, Performance Improvement, Patient Safety,
Accreditation Readiness, Clinical Data /Analytics and Abstraction,
etc). Minimum of 5 years of progressive leadership experience
required, preferable in a healthcare, post-acute setting
- South Carolina RN Nursing Licensure
Other Required Skills/Experience:
- Professional Certification preferred (i.e., CPHRM, CPHQ, CJCP,
CPPS, etc. )
- Group facilitation and public speaking - Required
