Job SummaryThe RN-Patient Safety Specialist is responsible for coordinating and implementing a collaborative and proactive patient safety program, promoting the culture of safety and high reliability, developing, implementing, improving, and evaluating patient safety systems. The position is responsible for supporting continuous patient safety improvement and ensuring sustained adherence to established patient safety program elements, standard work practices, regulatory requirements and accreditation standards. In partnership with Risk Management, the Patient Safety Specialist is responsible for reviewing event reports, performing investigations and risk assessments and identifying and communicating patterns and trends that will facilitate improvement. Other key responsibilities of the Patient Safety Specialist include:

--- Serving as a resource for development and implementation of patient safety policies and activities

--- Identifying methods for reduction of error and other factors that contribute to adverse outcomes

--- Facilitating safety initiatives using a collaborative, proactive approach and connecting with front-line staff and key stakeholders to identify barriers to care and areas of patient safety opportunity

--- Providing guidance, consultation and standard work for the design and maintenance of a comprehensive patient safety program

--- Communicating evidence-based ideas regarding effective patient safety strategies and techniques

--- Working in collaboration with Human Resources, Environmental Safety, and other leaders to develop and assure a culture of safety in the workplace.

The position supports the campus/site clinical and administrative leadership team, quality, risk management, accreditation, and infection preventionist colleagues, related Subject Matter Experts (SME), stakeholder groups, and ad hoc teams on patient safety related performance improvement activities. The scope of the role may apply to the ambulatory, acute and/or post-acute care settings.Job DescriptionAccountabilities


  • Provide direct support to the development, implementation, and operation of the patient safety program and standard work processes. Establish standard work and participate in efforts for concurrent monitoring as needed for compliance and results based on new implementations or processes under review. Establish mechanisms for proactive identification and correction of patient safety issues.
  • Lead safety huddles and perform safety rounds to proactively identify safety risks and hazards and evaluate processes to assure adherence to the patient safety program plan and standard work processes. Recommend enhancements for improving patient, team member, and environmental safety. Proactively addresses questions or concerns involving patient safety.




    • Complete timely review of occurrence reports submitted by hospital personnel and or medical staff for the purpose of identifying patterns or trends. Leads opportunities for improvement and support efforts to prevent future events. Work in collaboration with leadership and team members to ensure timely reporting and management of patient safety issues and findings at the operational level. Provide and communicate patient safety trend reports to stakeholders and leaders, in addition to assisting in the development and monitoring of action plans.
    • Partner with Quality and Risk Management to facilitate failure mode effects analyses and identify high risk processes and systems to proactively promote safety and error prevention. Utilize information from external reporting programs to mitigate risk (i.e. Institute for Safe Medication Practices (ISMP) Medication safety alerts, Emergency Care Research Institute (ECRI), Joint Commission Sentinel events). Develop and oversee implementation of improvement action plans.
    • In partnership with Quality and Risk Management, leads a thorough root cause analysis in the event of a serious safety or near miss event using robust process improvement tools and techniques. Develop improvement plans. Assist operational leaders with implementation of relevant strategies and disseminate lessons learned from RCA findings and recommendations across the organization.
    • Assist with the deployment of the AHRQ Culture of Safety Survey and work with leaders on development and monitoring of improvement plans.
    • Assist with implementation of reliable systems to support evidence-based care to improve clinical outcomes and patient safety.
    • Partner with Quality and Risk Management to integrate and assimilate information obtained from the reporting of adverse events, near misses, root cause analyses, and clinical and operational issues related to risk. Analyze findings and works collaboratively with key stakeholders to develop and implement programs that will enhance an organizational culture that supports safety and reduces risk. Prepare, provide, and participate in the reporting of patient safety related information to Leadership, Governing Board, and other applicable committees, per schedule and on an as needed basis. Participate in annually reviewing the effectiveness of the Patient Safety program and contribute to organizational goal setting and prioritization of initiatives at least annually.
    • Contribute to deploying a patient safety curriculum for the organization that supports evidenced based practices including High Reliability and Culture of Safety concepts. Develop and participate in patient safety orientation and educational endeavors, annual recurrent training, one-on-one consultations, and in-service training.
    • Serve as an internal consultant on patient safety related subjects. This includes providing direct support to programs that involve patient safety. Participate in the development of administrative and clinical policies regarding patient safety. Work with all stakeholders to implement programs designed to maximize patient safety performance and evaluation as established by the National Quality Forum (NQF) and The Joint Commission (TJC) National Patient Safety Goals (NPSG) and Sentinel Event Alerts. Conduct training and coaching, and develop templates, tools, and processes to facilitate an efficient and effective means for patient safety improvement. Disseminate patient safety best practices and educational materials and communicate evidence-based ideas regarding effective patient safety strategies throughout the organization.


      Supervisory/Management ResponsibilitiesThis is a non-management job that will report to a supervisor, manager, director or executive.Minimum EducationBachelor's Degree - Nursing, health care-related field, health administration, business administration or related field.       Required Certifications/Registrations/LicensesRN - REGISTERED NURSE - (Code NLRN)Other Required ExperiencePreferred - Certified Professional in Healthcare Risk Management (CPHRM), Associate in Risk Management Certification (ARM) or like certification (e.g., Certified Healthcare Safety Professional (CHSP); Certified Professional in Patient Safety (CPPS)).
      Preferred- Hospital/health care patient safety experience.
