Job Summary

The Patient Care Facilitator (PCF) provides clinical leadership for a team of nurses and patient care technicians assigned to a specific geographically designated area of the unit (patient care center). The primary role of the PCF is to know each of the patients within their assigned area acting as liaison for physicians, nurses, and other members of the healthcare team in the care of the patient. The PCF assists in the development and implementation of patients care plan and coordinates timely and appropriate discharge. The PCF coordinates all patient care ensuring continuity and is the one consistent person that the patient and family see during the hospital stay. The PCF provides excellence in bedside caring as well as clinical expertise and acts as patient advocate throughout the course of their hospital stay. The PCF monitors quality of care and mentors staff within their care center. The PCF has 24 hour accountability for patients within their care center.

Accountabilities

* CLINICAL EXCELLENCE is evidenced by the following:
* Directs and participates in the care of patients;
* Coordinates the communication within the team, the other disciplines and physicians;
* Serves as a clinical resource for nursing staff, unlicensed assistive personnel and physicians;
* Participates in determining plans of care which are inclusive of patient outcomes resulting from nursing interventions;
* Promotes use of evidenced based standards and practice at the unit level and at other sites as appropriate.
* Demonstrates competence in skills for direct patient care
* Identifies clinical issues and initiates change related to improving care or processes through leadership of PI teams

Insures compliance with identified quality indicators - 30%

* FINANCIAL PERFORMANCE - Functions as change agent and leader as evidenced by the following:
* Collaborates with leadership to achieve targeted financial indicators, i.e. LOS, practice changes---
* Implements cost effective initiatives in delivery of quality patient care,
* Provides leadership for performance improvement teams that have potential for cost saving initiatives and clinical improvement in practice and work flow,
* Reviews variance data to identify opportunities for improvement;
* Investigates ways to minimize cost while maintaining excellence in practice.
* Impacts LOS by coordinating and overseeing discharge planning - 20%


* LEADER/EDUCATOR: Functions as a role model and clinical leader as evidenced by the following:
* Serves as a role model for nursing staff to improve clinical skills and professionalism
* Works closely with management team in planning clinical goals and objectives for patient unit
* Contributes to the orientation process of other healthcare personnel (i.e. PCT's, Unit secretaries, surgical techs---)
* Serves as a Frontline clinical leader and role model for staff and is permitted and expected to exercise independent judgment within their roles
* Mentors new and experienced nurses for professional development - 20%


* PATIENT & CUSTOMER SATISFACTION
* Promotes integration of patient & family centered care concepts, ethical standards, diversity and promotes the focus on customer satisfaction into care delivery
* Contributes to the educational process for nursing students and other students in the clinical area, as appropriate
* Exhibits leadership skills in facilitating improvements in customer satisfaction
* Liaisons with physicians to strengthen professional partnerships - 20%


* PROFESSIONAL DEVELOPMENT: From the following list, A is required, plus at least two others:

A.Fifteen (15) continuing education hours in area of specialty related to role or clinical practice (excluding mandatory annual training) comprised of 10 hours in speciality area and at least 5 hours in professional development training (i.e., leadership skills, time management, etc.)

B. Active participation in local or national chapter of professional specialty organization

C. Community healthcare participation

D. BCLS or ACLS instructor

E. Poster presentation at national/regional/local seminar

F. Presentation at national/regional/local seminar in specialty area

G. Healthcare academic education

H. Obtains or maintains certification through an approved national certification board

I. Present 2 unit based in services - 10%



Supervisory/Management Responsibilities

This is a non-management job that will report to a supervisor, manager, director or executive.

Minimum Requirements

* Associate's Degree in Nursing
* 3 years - Acute care in specialty area

In Lieu Of

In Lieu of the education and experience requirements noted above, will except 18 months experience in specialty area along with related specialty certification.

Required Certifications, Registrations, Licenses

* RN - REGISTERED NURSE - NLRN

Knowledge, Skills or Abilities

* Basic computer skills including word processing and data entry
* Mathematical skills
* Patient Equip (Suction, Vitals, Defib)
* BSN- Preferred


* BCLS- Required
