• Collects data and completes nursing assessments to inform individualized patient plans of care.
• Develops and implements effective care plans that address the unique needs of each patient.
• Collaborates with interdisciplinary healthcare teams to ensure seamless coordination of care and achieve optimal patient outcomes.
• Provides education and support to patients and their families to promote health literacy and self-management skills.
• Evaluates the effectiveness of care plans and makes adjustments as needed to ensure continuous improvement.Requirements
  • Associate's degree in nursing (BSN preferred)
  • One year of experience as a registered nurse
