About Us Our organization has been a pioneer in establishing the
first national in-home clinical network, currently functioning as
an independent provider of extensive health assessments for
individuals nationwide. We are dedicated to delivering outstanding,
personalized care that enhances health, reduces costs, and empowers
individuals to lead fulfilling lives. With a strong foundation in
clinical assessment and care management, our nationwide network of
clinicians transcends traditional barriers by meeting members in
their own homes. We assist older adults and other at-risk
populations in achieving better care quality, improving health
outcomes, and identifying chronic conditions that might otherwise
remain undetected. Our available positions enable you to utilize
your skills and compassion to significantly influence the health
and well-being of others. Join our team and enjoy competitive pay
along with flexible scheduling while making a positive impact in
the community! Responsibilities About the Role During visits that
may last up to one hour, our providers assess and observe a
member’s current health status, medical history, medication
adherence, social environment, and other potential risks. This
approach provides invaluable insights into a member’s overall
health that are often missed during standard office visits. The
Matrix Comprehensive Health Assessment (CHA) enhances care quality
and allows us to address multiple care gaps in a single visit. Our
providers are responsible for: Conducting assessments for adults
and geriatrics, including medical history, diagnosis and treatment,
health education, physician referrals, case management referrals,
follow-up, and thorough documentation in line with our guidelines
and protocols. Administering and ordering necessary point-of-care
and lab-based tests. Collaborating with physicians, case managers,
social workers, family members, key caregivers, and other medical
personnel as appropriate. Working alongside Primary Care Physicians
(PCPs) for patient education and necessary follow-ups. Providing
services at the member’s residence, which may include their homes
or Skilled Nursing Facilities. Benefits of Working with Us
