This role is pivotal in enhancing the quality of patient care by ensuring comprehensive coordination of healthcare services. Working in our Program of All-Inclusive Care for the Elderly (PACE), you will manage a range of tasks from assessing patient needs to implementing health services. Your dedication will aid in achieving optimal health outcomes for our elderly patients by facilitating coordinated care plans that meet their unique needs. Assess patient health needs and medical history Develop and implement comprehensive care plans in collaboration with a multidisciplinary team Coordinate with healthcare providers and external agencies to ensure timely delivery of services Monitor patient progress and adjust care plans as needed Educate patients and families about health management and resources Ensure compliance with healthcare standards and safety regulations Strong clinical skills in a healthcare setting Excellent communication and interpersonal skills Ability to work effectively in a multidisciplinary team Detail-oriented with strong organizational skills Experience in case management and patient care coordination Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current Registered Nurse (RN) license Certification in Case Management (CCM) preferred but not required
