Job Description:
As a dedicated RN Care Coordinator PACE at HCA Healthcare, you
will have the unique opportunity to enhance the quality of care for
our elderly patients enrolled in the Program of All-Inclusive Care
for the Elderly (PACE). Positioned at the intersection of
compassionate patient interactions and the utilization of
healthcare resources efficiently, you are tasked with coordinating
comprehensive care plans that address the multifaceted needs of
patients. In this role, you will oversee assessments, plan
healthcare services, advocate for patient needs, and ensure that
care delivery is both efficient and effective. Evaluate the
healthcare needs of PACE enrollees through comprehensive
assessments. Develop, implement, and continuously update
individualized care plans. Coordinate with a multidisciplinary team
to ensure integrated and effective care delivery. Monitor patients’
health status and respond to changes in their condition. Educate
patients and families on health conditions and care needs. Manage
healthcare resources efficiently to ensure cost-effective care.
Advocate for patients' access to required services. Document all
patient interactions and interventions accurately. Strong clinical
skills in a healthcare setting Excellent communication and
interpersonal skills Proven ability to work effectively in a team
Skills in patient assessment and care planning Ability to manage
multiple priorities in a high-paced environment Proficient in
healthcare IT systems Medical Insurance Dental Insurance Vision
Insurance Life Insurance 401k Current Registered Nurse (RN) license
in the state of practice Certified in Basic Life Support (BLS)
Certification as a Case Manager (CCM) preferred or willingness to
obtain
