Job Summary

naviHealth is searching for a Registered Nurse to join our new Institutional Special Needs Plan team (I-SNP). In this position, you will assist with the development and execution of the clinical model for the I-SNP members alongside Nurse Practitioner Care Managers and in collaboration with the Primary Care Provider. The RN Care Manager will partner with the NP Care Manager to provide the highest quality of care to I-SNP members.

Responsibilities

  • Assists with the development, maintenance, and execution of care plans for members
  • Engages in meaningful Advance Care Planning discussions with members to establish goals of care and assists in the execution of appropriate legal documentation to support their end-of-life wishes
  • Conducts ongoing assessmentsof low risk I-SNP members
  • Responds to acute visit requests from the nursing facility staff, in collaboration with the NP Care Manager and the Primary Care Provider
  • Identifies and coordinates needs, resources, and other key elements required for member care management and coordination of care
  • Visits members routinely, and in coordination with the NP Care Manager, as indicated by their care plan
  • Collaborates with ICT to identify and address member needs
  • Advises on appropriateness of transitions of care through consultation with the NP Care Manager,facility RN/LPN and can be the first point of contact for the facility RN/LPN
  • Provides care to members when deemed clinically necessary and/or to support the ICT in times of heightened demand
  • Develops relationships with members of the ICT both in the facility and in the community such as local health systems, hospitals, skilled nursing facilities, rehab facilities, and PCP offices 
  • Provides clinical and non-clinical education to members and their families, including self-management techniques
  • Provides care coordination support with I-SNP facility staff during transitions of care 
  • Evaluates inpatient treatment and clinical progress of members admitted to acute care facilities, skilled nursing facilities and inpatient rehabilitations facilities and coordinates continued services during transition back into their institutional or institutional-equivalent setting
  • Facilitates family care conferences to address medical, behavioral, and social needs
  • Makes rounds with the NP Care Manager, assists with information gathering
  • Makes independent rounds in the nursing home, checking in on patients, identifying changes in condition, following up on treatments in place, checking in with nursing staff
  • Assist as needed with facility education and inservicing
  • Perform other duties and responsibilities as required, assigned or requested
  • This position will have access to protected health information (PHI) to perform their routine job functions and will be expected to safeguard the confidentiality and integrity of such data in accordance with company policy and applicable federal and state regulations.

Qualifications  

  • Bachelor’s degree plus state specific Registered Nurse licensure
  • Experience in Geriatrics, Hospice, Palliative Medicine, or the long-term care population preferred
  • Knowledge of care management
  • Intense intellectual curiosity and an ability to view old problems with a fresh perspective
